If You Have Diabetes and You're on Medicare...

You may be eligible for some benefits that could improve your life. The key to success with diabetes is taking charge of your condition and having the knowledge to manage it day by day. Lehigh Valley Hospital and Health Network offers these vital programs to help you:
  • Diabetes Self-Management Training—The Helwig Health and Diabetes Center teaches you the basics about diabetes as well as the latest on blood glucose monitoring, medications, nutrition, exercise, and how to prevent and treat complications. Medicare covers 10 hours of training the first year and two hours of follow-up each year thereafter.
  • Medical Nutrition Therapy—In this companion program to Self-Management Training, you get individualized visits from a registered dietitian to fine-tune a meal plan that matches your needs. Medicare covers three hours the first year, two hours a year thereafter.
People in diabetes education programs tend to need less medication, have fewer hospitalizations, and reduce their risk for dangerous complications such as heart disease. Both these programs require a doctor’s referral, and you’ll need to check your eligibility.

Medical Nutrition Therapy

If you're on Medicare Part B and have diabetes or non-dialysis kidney disease, you're eligible for one-on-one dietitian visits to help lower blood glucose and cholesterol. Meal planning, carb counting, shopping tips, label reading. Doctor referral needed. Appointments available weekdays at all three Lehigh Valley Hospital and Health Network sites.
